Please read the following carefully before submitting your booking form. If you have any questions or are unsure about anything, contact me before agreeing.

I confirm that the information I have provided in this form is accurate to the best of my knowledge. I understand that I should tell Arkay about any relevant change to my health, medication, skin condition or other information before my appointment.

I understand that tattooing permanently places pigment into the skin and may involve pain, bleeding, swelling, bruising and irritation. I understand that complications such as infection, allergic reaction, scarring or other unexpected reactions are possible, and that individual healing and final results can vary.

I understand that the way a tattoo heals can affect the final result. I agree to follow the aftercare instructions provided to me and to contact Arkay if I have concerns about the healing of my tattoo. I understand that medical concerns should be assessed by an appropriate healthcare professional.

I understand that I must arrive able to give informed consent and must not attend my tattoo appointment intoxicated or impaired by alcohol or recreational drugs.

I understand that Arkay’s standard tattoo rate is $200 per hour including GST unless another rate or project price has been agreed with me in writing.

I understand that a 25% deposit is required to secure my booking and begin the design process. Deposits are non-refundable. A deposit may be transferred to a rescheduled appointment when at least 48 hours’ notice is given.

I understand that my appointment is not fully secured until the required deposit has been received.

I voluntarily consent to Arkay carrying out the tattoo described in this booking form at the agreed placement. I understand that I can ask questions before tattooing begins and that I can withdraw my consent before or during the procedure.

Photo & Portfolio Permission

Do you give Arkay permission to photograph or film your tattoo and use those images or videos in portfolios, social media, websites, promotional material or other examples of his work? Choosing No does not affect your booking or tattoo.
